NNMC Autumn Enrolment (Half-term)

We need details for the main point of contact for us to share information with, and also to contact if any issues arise during our sessions.

We will only ring you with short notice changes to the programme, or in an emergency. We will use email to send you useful information and updates.

If you feel that there is information that you can provide that will help your child flourish at New Noise Music Centre, please detail in the relevant sections below.

If there is anything you feel you'd rather discuss in person, please drop faye@futureyard.org an email to arrange.

Please tell us any additional information that will help us to support your child. E.g. does your child have any disability, access needs or special education needs?

Please tell us about any medical conditions or allergies we need to consider; or if your child has experienced any mental health issues you think it would be beneficial that we know about, such as low mood or anxiety.

From time to time we capture our New Noise Music Centre activity through photos and videos. These are used for the following:

To evidence activities that are necessary for Arts Award Bronze (this is shared with the Arts Award moderators); To share on Future Yard socials to celebrate successes and shout about our programme; In marketing materials that are used to advertise available places on our New Noise Music Centre programmes.

There is a £32.50 fee per child for the remainder of the term. There are a number of exemptions listed below: if you fall under any of the below categories, please tick the box(es) which apply.

We appreciate that circumstances can often change, so if you do face barriers to paying this fee, please contact faye@futureyard.org and we will try our best to accommodate.
